Richfield Health Center – Salem was the second largest nursing home in Salem City County in the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported.

The facility offered 112 available beds—20 fewer than the county average for that period—and cared for an average of 101.6 residents during the quarter.

CMS rated Richfield Health Center – Salem with an overall score of 3 out of 5, matching the statewide average rating for nursing homes in Virginia.

Salem Health & Rehabilitation led the county in size, with 240 available beds, while Snyder Nursing Home ranked third, offering 45 beds.

Richfield Health Center – Salem faced $15,465 in fines and received one penalty as of the first quarter of 2026.

At that time, Salem City County, Virginia, included two non-profit nursing home facilities.

U.S. Census Bureau estimates show the number of older adults continues to climb while younger age groups decline. Between 2020 and 2024, the population of Americans age 65 and older grew by 13%, as the group under age 18 decreased by 1.7%. Over the last 100 years, the nation’s older adult population expanded sharply, rising from 4.9 million in 1920 to above 61 million by 2024.

A 2021 study found almost half of this demographic—about 28 million people—may need long-term services and supports. The research also projects that over a third of older adults could eventually require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an education-focused organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
